Job Summary:  This position will help support all aspects of care within the organization's and provider's scope of practice.  The NP will provide medical care to patients as outlined in their collaborative agreement under the oversight of their supervising physician in the outpatient, inpatient, home, or nursing home setting.  Knowledge and advanced skills with advanced physical assessment and treatment of complex wounds, management of skin integrity and chronic care management are required.

Detailed Responsibilities:

  • Evaluates and sets a treatment plan for all initial and established wound referrals at assigned care locations.
  • Accurately performs history and physical examinations on adult patients with acute and chronic wounds.
  • Assess, diagnose, and manage chronic conditions for patient population.
  • Identifies deviations from normal, initiates, and evaluates treatment for patients with commonly occurring health problems within scope of practice.
  • Consults, co-manages, or refers as appropriate.
  • Prescribes medications as delegated in the protocols and/or procedures.
  • Provides individualized patient teaching and counseling.
  • Communicates with PCP or office staff regarding any changes in the patient’s condition.
  • Communicates with collaborating provider as outline in collaboration agreement and state guidelines.
  • Completes necessary documentation as outlined in hospital and RXH Policy.
  • Develops, implements and/or participates in research or educational activities.
  • Maintains competency in clinical practice and meets and maintains requirements for all certifications.
  • Participates in quality improvement activities and monitoring of health outcomes for patients.
  • Ensures proper documentation of diagnoses and appropriate coding.
  • Assess and creates treatment plans and prescribes medication in accordance with state requirements and organization protocols.
  • Initiates evaluation and treatment procedures essential to appropriate response to emergency wound situations.
  • Assure staff knowledge and practice surrounding goals of healthy skin integrity and wound management.
  • Actively participate in creating and implementing improvements to achieve clinical, satisfaction and/or efficiency outcomes.
  • Participate in education of patients as well as hospital, nursing home and RXH staff to assure best outcomes and most current clinical practice.
  • Work to ensure the continuity of the mission, vision, and values of the hospital.
  • Responsible for ensuring current state license
  • Other duties as assigned.

Education and Technical Skills:

  • Masters or Doctoral Degree as a Nurse Practitioner
  • Valid state nursing license
  • 2 years’ experience working in an acute care setting as an APRN or RN in one of the following departments: Wound Care, Emergency Room, and or Medical/Surgical Unit
  • Excellent interpersonal skills
  • Board Certification in any of the following Adult Nurse Practitioner/ANP, Family Nurse Practitioner/FNP, Geriatric Nurse Practitioner/GNP, or Clinical Nurse Specialist/ CNS Active DEA
  • Prescriptive authority
  • BLS from American Heart Association

Additional Eligibility Qualifications/Competencies:

  • Strong knowledge base of HIPAA and OSHA required
  • Patient, compassionate nature
  • Ability to work within a team and take direction
  • Ability to operate as part of a team, alongside physicians and other nurses
  • Proficiency in medical instruments and equipment

  • Must be DOT certified or willing to complete DOT certification within first 3 months of employment.
